Who Pays My Bill?

Medicare

We will bill your Medicare Part B. If you receive a signature form from us in the mail, please complete and return it to us to ensure prompt processing. Some of our services, including wheelchair transportation, are not covered by Medicare. You will be responsible for this bill. Please contact our office if you have any questions. If your ambulance claim was denied, you can appeal your claim with Medicare.

Medicaid

If you received a bill, and we should be billing Medicaid, please contact our office immediately or provide us with your insurance information with our Insurance Request Form.

No-Fault

If you were involved in an automobile accident, make sure you notify your insurance agent immediately and, after doing so, provide us with your No-Fault insurance.

Workers Compensation

If you were injured on the job, and you received a bill, please complete the Insurance Request Form with your worker's compensation information. If you do not know this information, please contact your place of employment for it.

Commercial Insurance

If you are covered by a traditional insurance program, please take a look at your invoice to see if we have billed it. If we have not, please complete the Insurance Request Form and return it to us. We will gladly submit the claim to your insurance.

Private Pay (No Insurance)

If you are unable to pay your bill in full at this time, please contact our office. We understand that medical bills are typically unforeseen expenses, and we will be happy to work with you on creating a payment plan.

We respect and honor the rights of all patients by thoroughly investigating any grievance you may have.
